A Diablo 4 fountain that turns to blood without opening an accessible red portal has not completed entry activation; stop spending Wirt's Lucky Coins and check access before trying again. Unlike D4 Gold, fountain entry depends on specific seasonal materials and progression. A missing entrance, interrupted seasonal progression, and a clear that awards only a trophy need different responses.

Check Old Tristram Access Before Spending Another Coin

Old Tristram requires all ten Deckard's Journals and their resulting progression, not simply a completed Prime Evil dungeon. Wirt's Lucky Coins are fountain activation materials, so spending them cannot replace the journal requirement.

Complete the seasonal introduction and advance the seasonal questline before deliberately changing Prime Evil powers.
Find and read all ten Deckard's Journal satchels across Sanctuary. Watch for the nearby journal marker rather than relying only on ordinary quest markers.
Follow the location path Caldeum > Imperial Library > Deckard's Memory > Old Tristram. Confirm that the memory actually takes your character into the town.
Check that you have one Splinter of Terror, one Splinter of Destruction, and one Splinter of Hatred as physical consumable items.
Combine those three different items in the Horadric Cube to obtain Wirt's Lucky Coins. Enter Old Tristram on a suitable Torment difficulty before using a coin at the fountain.

The three Prime Evil Splinters serve both power selection and the hidden recipe, so activating one removes an item you intended to save for entry. Jewelry Soul Splinters belong to a separate socket system; removing those does not supply the recipe materials.

Successful fountain activation changes the water to blood and produces a red portal leading to Echo of Tristram. The destination contains the Echoes of Diablo, Baal, and Mephisto together. Blood-colored water alone is not confirmation that you can enter.

Match the Recovery Step to the Actual Symptom

A consumed coin without a usable portal is an entry failure, while receiving only the Standard of Heroes after a clear is a reward issue. Check which event occurred before changing your setup or committing more materials.

The fountain changes, but no portal is accessible

Stop activating the fountain in that same situation. Record the selected difficulty, party state, activation time, and fountain appearance before leaving, and note whether the coin disappeared.

Those details help describe the failure accurately, but they do not identify a guaranteed repair. No universally reliable recovery method is established for this symptom, so another immediate coin payment is not a dependable fix.

Seasonal progression stopped after early Splinter use

Patch 3.2.3 corrected a Prime Evil Splinter interaction that could interrupt the seasonal questline, but it did not automatically repair affected characters. Its recovery route requires another character in the same Normal or Hardcore and standard or SSF partition.

Complete the seasonal questline on that other character to unlock Prime Evil Powers for characters in the matching partition. This does not reset the original character's quest progress, so do not expect its quest state to return to the beginning.

The encounter finishes, but only a trophy appears

The Standard of Heroes is a completion trophy, not a damage upgrade. The Hellfire Torch is the Mythic Charm associated with the encounter, and receiving the trophy alone does not establish that another immediate clear will provide the charm.

Closely repeated clears have been associated with trophy-only rewards, but no dependable reward interval is established. Treat that uncertainty as a reason to preserve coins rather than repeatedly paying to test a timer.

Stop When the Next Attempt Cannot Answer Anything New

Stop after a consumed coin produces no accessible portal, or after a recent clear leaves you considering another entry solely to test reward timing. There is no confirmed waiting duration to recommend for either situation; an exact minute count would be guesswork.

Use the pause to check combat readiness instead. Keep a defensive skill and an escape option, and confirm that you can consistently clear the individual seasonal boss encounters before paying for the combined fight.

Before spending time on D4 Gold for sale, return to the Imperial Library and verify that Deckard's Memory opens Old Tristram. Preserve one of each recipe Splinter, then reserve your next coin for an attempt with confirmed access and a prepared bossing setup.
